Personal Support Worker (PSW) Responsibilities:

  • Assist residents with their support care needs and implement their Support Care Plans.
  • Facilitate and participate in residents' daily living activities as outlined in their Individual Support Plan.
  • Ensure residents follow their prescribed dietary needs and encourage social and recreational participation.
  • Provide physical assistance and ensure safety using mobility devices.
  • Perform laundry duties as needed and continuously assess and report unsafe conditions.
  • Comply with end-client standards, government policies, and regulations in daily activities.
  • Complete and file reports, handle complaints according to standards, and perform tasks delegated by RNs and supervisors.
  • Communicate effectively with co-workers, maintain confidentiality, and report conflicts or concerns immediately.
  • Adhere to agency procedures, occupational health and safety standards, and the universal code of conduct for personal support workers.
  • Inspect equipment before use, report concerns, and follow safe operating procedures.
  • Stay updated on current procedures and maintain relevant certifications (First Aid/CPR, Crisis Prevention).
  • Attend required training sessions and be available to work on weekends and weekdays.

Qualifications:

  • Personal Support Worker (PSW) certificate
  • Ability to speak in Cantonese and/or Mandarin is preferred
  • At least 1 year of previous experience in patient care, senior care, or caregiving roles preferred
  • Strong communication skills and empathy towards those in need of care
  • Ability to work effectively in a team and follow instructions from healthcare professionals

Registered Practical Nurse (RPN) Responsibilities:

  • Perform initial and ongoing assessments of residents' physical, emotional, and mental health.
  • Monitor vital signs (blood pressure, pulse, temperature, respiratory rate).
  • Administer prescribed medications and treatments, including injections and IVs.
  • Monitor and document residents' responses to medications and treatments.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Adjust care plans based on residents' needs and condition changes.
  • Assist with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility.
  • Provide wound care and other specialized nursing procedures.
  • Use PPE and maintain a clean and safe environment.
  • Adhere to College of Nurses of Ontario (CNO) standards and ethical guidelines.
  • Engage in continuous professional development and stay current with nursing advancements.
  • Maintain good standing with the CNO and report any CNO complaints or disciplinary actions.
  • Communicate with residents’ families, providing updates and addressing concerns.
  • Complete and file reports according to LTC facility standards and government regulations.
  • Maintain confidentiality of all information related to the agency and residents.
  • Report conflicts or concerns to management immediately.
  • Follow policies and procedures in accordance with the Occupational Health and Safety Act of Ontario.
  • Inspect equipment before use and report any concerns.
  • Follow safe operating procedures for mechanical equipment.
  • Stay updated on current procedures, treatments, and techniques through approved courses.
  • Attend all Health and Safety and required workshops/training sessions.
  • Maintain First Aid/CPR and Crisis Prevention and Intervention certifications.
  • Be available to work on weekends and weekdays.

Qualifications:

  • A valid certificate of registration from the College of Nurses of Ontario
  • Graduate from a college-level institution with a diploma in Practical Nursing
  • Previous experience in a healthcare setting, preferably in transitional care.
  • Strong clinical skills and knowledge of nursing practices.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a team-oriented environment.
  • Compassionate and patient-focused approach to care.

Registered Nurses (RN) Responsibilities:

 

  • Conduct comprehensive assessments of residents' physical, emotional, and psychological health.
  • Develop, implement, and evaluate individualized care plans with residents, families, and the interdisciplinary team.
  • Administer medications, monitor side effects, and adjust care plans as needed.
  • Regularly monitor and document residents' health conditions, vital signs, and treatment responses.
  • Collaborate with physicians, therapists, social workers, and other healthcare professionals for comprehensive care.
  • Communicate with families about care plans, condition changes, and treatment options.
  • Provide wound care and specialized nursing procedures.
  • Assist in scheduling and coordinating nursing staff for adequate coverage.
  • Monitor and manage medical supplies and equipment, ensuring proper usage.
  • Use PPE and maintain a clean, safe environment.
  • Adhere to CNO standards and ethical guidelines; maintain good standing with regulatory bodies.
  • Engage in continuous professional development to stay current with nursing practices.
  • Complete and file reports in accordance with LTC facility standards and government regulations.
  • Maintain confidentiality of all agency and resident information.
  • Report any conflicts, concerns, or CNO disciplinary actions to management immediately.
  • Follow Healthcare Job.Agency and client policies and procedures, including safety regulations.
  • Inspect equipment before use, reporting any concerns.
  • Attend all required Health and Safety workshops/training sessions.
  • Maintain certifications in First Aid/CPR and Crisis Prevention and Intervention.
  • Be available to work on weekends and weekdays.

Qualifications:

  • Completion of a recognized nursing program (Diploma or Bachelor’s degree in Nursing).
  • Current registration and good standing with the College of Nurses of Ontario (CNO).
  • Previous experience in long-term care (LTC) or nursing home settings preferred.
